Appointments & Referrals

  • HMO Plans: A written referral from your primary physician is required. If your insurance requires a referral and you do not have one, you may be asked to pay in full or reschedule.
  • Self-Pay Patients: A credit card is required to book. A $50 non-refundable fee will be charged to secure your spot; the remaining balance is due at check-in.
  • What to Bring: Insurance card(s), medical records/X-rays, a current medication list, and your payment method.
  • Arrival: Please arrive 15 minutes early for paperwork. If you are late, you may need to reschedule.

Cancellation & No-Show Policy

  • Notice Period: Cancellations must be made at least 24 hours in advance.
  • Fees: Late cancellations or “No-Shows” will result in a $50 charge to the credit card on file.
  • Security: Credit card information is stored securely according to industry compliance standards.

Payments & Insurance

  • At the Time of Service: All co-pays, deductibles, and co-insurance are due in full during your visit.
  • Unpaid Balances: Any existing balance on your account must be settled before you can be seen for a new appointment.
  • Accepted Payments: The office accepts credit/debit cards and NFC mobile payments. NO CASH PAYMENTS
